Short Stay Skilled Nursing and Rehabilitation Services:
Often times, individuals enter a health center following a hospital stay when they are in need of rehabilitation and/or skilled nursing care for complex medical needs.

Our specialized program, known as Transitions, is staffed with professionals ready to assist you in your transition from hospital to home.
Transitions’ rehabilitation program offers a full range of in-patient medical care, nursing services, and advanced rehabilitation programs in physical, occupational and speech tharapy. The program focuses on helping individuals progress quickly and successfully in their recovery and rehabilitation.
We offer Nintendo Wii activities as part of our program. It allows residents to enjoy playing golf, tennis, bowling, and more while
they are in rehabilitation.
With an interdisciplinary team consisting of physicians, nurse practitioners, nurses, pharmacists, licensed therapists, and dietitians, every individual’s care needs will be met, as will their social and emotional needs.
Upon admission, the social worker will begin the discharge planning process and assist in the coordination of any necessary services individuals may need upon their return home.
Transitions is licensed and certified to accept coverage by Medicare, HMOs, private insurance and workers compensation insurance.
